Gap Recalls Children's Coats; Toggles Can Break and Detach Posing Choking Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of product: babyGap Children's Coats

Units: About 19,200 in the U.S. and 3,900 in Canada

Retailer: Gap Inc., of San Francisco, Calif.

Hazard: The coats have toggle fasteners that could break and detach from the coat, posing a choking hazard to young children.

Incidents/Injuries: None reported.

Description: This recall involves three different children's coat styles with toggles including Rose Toggle Hoodie (style 600298), Pink Duffle Coat (style 600344), and Blue Plaid and Mojave Toggle Coat (style 600942). The coats were sold in infant sizes up to 24 months and are made of cotton, polyester, or polyester/acrylic. The toggles are light tan plastic and about 1 3/4 inches long.

Sold at: babyGap, GapKids and Gap Stores nationwide and at www.gap.com from October 2008 through April 2009 for between $20 and $50.

Manufactured in: China and Indonesia

Remedy: Consumers should immediately take the recalled coats away from children and return them to any babyGap, GapKids, or Gap store for a full refund. Consumers who return the coats before August 31, 2009 will also receive a 20 percent off coupon for a regular priced item. Contact Gap to return by mail if purchased online.

Jardine Announces Second Recall Expansion of Cribs Sold by Babies'R'Us; Cribs Pose Entrapment and Strangulation Hazards

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Jardine Cribs

Units: About 96,000 (320,000 units were previously recalled in June 2008 and 56,450 units were recalled in January 2009)

Manufacturer: Jardine Enterprises, of Taipei, Taiwan

Hazard: The wooden crib slats can break, creating a gap, which can pose an entrapment and strangulation hazard to infants and toddlers.

Incidents/Injuries: CPSC has received 31 incident reports of slats breaking, including two reports of children becoming entrapped in the gap created by the broken slat on the crib models identified below. In 10 of these incidents, consumers reported that their child broke the slat while in the crib. There was one report of minor injuries (bumps and abrasions.)

Description: This recall involves 7 models of Jardine wooden cribs with the date codes identified below. Cribs with other date codes are not affected by this recall. The date code and model number are printed on the label located on the inside of the bottom rail of the headboard or footboard.

Pumpkin Patch Recalls Hooded Girls' Raincoats with Drawstrings Due to Strangulation Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Pumpkin Patch Hooded Girls' Raincoats

Units: About 800

Distributor: Pumpkin Patch LLC, of San Mateo, Calif.

Hazard: The raincoats have a drawstring through the hood which poses a strangulation hazard to children. In February 1996, CPSC issued guidelines (pdf) to help prevent children from strangling or getting entangled on the neck and waist by drawstrings in upper garments, such as jackets and sweatshirts.

Incidents/Injuries: None reported.

Description: This recall involves the Pumpkin Patch hooded raincoat with drawstrings in girls sizes 2 through 10. The raincoat is a trench coat style and is red with pink polka-dots.

Sold at: Pumpkin Patch stores nationwide and on-line at www.pumpkinpatchusa.com from July 2008 through February 2009 for about $50.

Fisher-Price Recalls 3-in-1 High Chairs Due to Fall Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of product: 3-in-1 High Chairs

Units: About 24,000

Importer: Fisher-Price, of East Aurora, N.Y.

Hazard: The seat can fall backwards from high chair frame if the booster seat release is unlatched while the child is in the product. Also, the seat back can detach if not fully snapped in place, posing a fall hazard and risk of serious injury to young children.

Incidents/Injuries: The firm has received one report of a seat back detaching and child falling out, resulting in a skull fracture.

Description: This recall involves the 3-in-1 High Chair to Booster(tm), which converts from a high chair to a toddler booster seat. It includes a removable tray, height adjustment and folds for storage. The product number (P5369) is printed on the side of the seat, on a label on the seat pad, and on the product's packaging.

Sold exclusively at: Target stores nationwide from December 2008 through March 2009 for about $100.

Manufactured in: Mexico

Remedy: Consumers should stop using the recalled high chairs immediately and contact Fisher-Price for instructions and a free repair kit.

Rashes from Carter's "Tag-less" Labels

CPSC and Carter's Advise Parents of Rashes Associated with Heat Transferred, or "Tag-less," Labels

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) and Carter's, Inc., of Atlanta, Georgia, are advising parents and caregivers that they have received reports that a small percentage of babies and infants have developed rashes on the upper back after wearing Carter's clothing with heat-transferred, or "tag-less," labels.

This advisory applies to Carter's Fall 2007 product line. The Fall 2007 line utilizes a label on the inside back of the garment that has a raised surface with a solid, rather than a stenciled, background. This advisory does not apply to previous and current product lines, which utilize labels with stenciled backgrounds.

The garments, which were made in various countries, were sold at Carter's own retail stores and at department and national chain stores.

If your child develops a rash on the upper back after wearing garments that have a "tag-less" label with a solid background, you should stop using these garments. If the rash persists or worsens, you should contact your pediatrician. RECALL: Stroller Activity Bars Due to Choking Hazard

The U.S. Consumer Product Safety Commission, in cooperation with International Playthings of New Jersey, today announced a voluntary recall of about 10,000 Taggies™ Strollin’ Along Stroller Activity Bars. The shiny material on the elephant’s ear on the activity bar can detach, posing a choking hazard to young children. International Playthings has received three reports of the shiny material detaching and children putting it in his/her mouth. No injuries have been reported. This recall involves the Taggies™ Strollin’ Along stroller activity bars. The multicolored, fabric, and plastic activity bars feature a yellow giraffe, a purple hippo and a blue elephant attached to a 12” long elliptical base with straps that attach to a stroller. The Taggies™ and Earlyears® logos are located on woven and satin labels sewn into the seam on the lower left front of the activity bar. Sold at specialty toy stores nationwide and on the Internet from February 2007 through July 2008 for about $23 and manufactured in Hong Kong.

RECALL: Fisher-Price Recalls Learning Pots & Pans Toys Due to Choking Hazard

The U.S. Consumer Product Safety Commission, in cooperation with Fisher Price of New York, today announced a voluntary recall of about 15,000 Learning Pots & Pans™ Toys. Missing screws in the blue toy pan can cause the clear plastic cover to come loose and release small balls, presenting a choking hazard to young children. Fisher-Price has received five reports of the plastic covers detaching and releasing the small balls. No injuries have been reported. This recall involves Learning Pots and Pans™ toy sets with stackable pots and pans, a lid and shape-sorting blocks. The toys have light and sound features that operate on three AA batteries. Model number G6685 is located on the bottom of the blue pan. They were sold at discount department and toy stores nationwide from October 2007 to August 2008 for about $20 and manufactured in China
Consumers should immediately take these recalled toys away from children and examine the bottom of the blue pan. The pan should contain six screws. If all six screws are installed, no further action is necessary. If any screws are missing, the consumer should contact Fisher-Price to arrange for the return of the blue pan for a replacement. RECALL: Lip Gloss & Jewlry Set Sold Exclusively at Wal-Mart Due to Rick of Lead Exposure

The U.S. Consumer Product Safety Commission, in cooperation with FAF Inc of Rhode Island, today announced a voluntary recall of 30,000 “Faded Glory” Lip Gloss, Locket, and Bracelet Sets. The lobster claw clasp on the bracelet contains high levels of lead, which is toxic if ingested and can cause adverse health effects. Injuries: None reported. The recalled three-piece sets contain a rectangle-shaped lip gloss container, a heart-shaped locket necklace, and a charm bracelet. The following models are included in the recall:

RECALL: Bayside Furnishings Recalls Youth Bed Toy Chests Sold At Costco After the Death of a 22-month-old Child

The U.S. Consumer Product Safety Commission, in cooperation with Bayside Furnishings of San Diego, Calif., on July 3rd announced a voluntary recall of about 9,350 LaJolla Boat Bed and Pirates of the Caribbean Twin Trundle Beds (pictured). The lid supports on the toy chests fail to prevent the lid from closing too quickly, posing an entrapment and strangulation hazard to young children. The Commission received one report of a death involving a 22-month-old boy of Roseville, California. He strangled when the lid of the toy chest fell on the back of his head and entrapped his neck on the edge of the chest of a LaJolla Boat Bed. This recall involves two styles of Bayside youth beds: the LaJolla Boat Bed and the Pirates of the Caribbean Twin Trundle Bed. The preassembled toy chests are designed in the shape of a ship or boat’s “bow” and attached to the beds as a footboard. The LaJolla Boat Bed toy chest has a hardwood top and white wood base with a blue stripe. The Pirates Boat Bed toy chest has a hardwood top, wheel shape and brown wood base with decorative carvings. They were sold at Costco and furniture retail stores nationwide and Costco.com from January 2006 through May 2008 for between $700 and $1,400 and manufactured in China.
Remedy: Consumers should immediately stop children from using the recalled toy chests and contact the firm for instructions on receiving a free repair kit with replacement lid supports.

RECALL: Children's Jewelry By Bead Bazaar Due to Lead Exposure

The U.S. Consumer Product Safety Commission, in cooperation with Bead Bazaar of Rockville, Maryland, on July 3rd announced a voluntary recall of 13,000 “It’s a Girl Thing” Bracelets, Necklaces, and Phone Charms. The children’s jewelry could contain high levels of lead. Lead is toxic if ingested by young children and can cause adverse health effects. Incidents/Injuries: None reported. This recall involves “It’s a Girl Thing” metal necklaces, bracelets, and phone charms. The jewelry features a variety of charms including flowers, shoes, letters and butterflies. “It’s a Girl Thing” is printed on the packaging along with the UPC numbers 633870018419 (bracelet), 633870018426 (necklace), and 633870018433 (phone charm).
Sold at retailers nationwide from February 2006 through June 2008 for about $6 and manufactured in China.

RECALL: Infants Garmets Due to Choking Hazard

The U.S. Consumer Product Safety Commission, in cooperation with Rashti & Rashti of new York, on July 1st announced a voluntary recall of about 6,200 Taggies™ Sleep’n Play Infant Garments. The snaps on these garments can detach, posing a choking hazard to young children.
Incidents/Injuries: None reported. The recall involves two Taggies™ Sleep ‘n Play style infant garments sold in purple or blue and white with style number listed in chart. The infant garments are one piece footed coveralls with snaps down the front. These garments were sold in sizes 0-3 months, 3-6 months, and 6-9 months. RN #27829 is printed on the care label sewn into the garment. Style numbers are printed on the hangtag of the garments.

RECALL: Stanley Furniture Cribs Due to Entrapment Hazard

The U.S. Consumer Product Safety Commission, in cooperation with Stanley Furniture Company of Virginia, today announced a voluntary recall of about 1,200 2nd Nature Built to Grow Cribs. The cribs could fail to meet a federal safety standard for crib dimensions. When the mattress support is in the middle setting, the space between the mattress and the crib could be too wide, posing an entrapment hazard to infants. Incidents/Injuries: None reported. This recall involves the full size 2nd Nature Built to Grow cribs, model number 353-14-220. Model and serial numbers are located on the headboard. The crib is natural maple in color. Only the following serial numbers are included in the recall: 353-14-220-L-01, 353-14-220-L-02, 353-14-220-L-03, 353-14-220-L-04, 353-14-220-L-05, 353-14-220-L-06, 353-14-220-L-07, 353-14-220-L-08, 353-14-220-L-101, and 353-14-220-L-103. The recall does not include cribs sold after December 2007. They were sold at department and furniture stores from March 2006 through December 2007 for about $1,100 and manufactured in Slovenia.
Consumers should immediately stop using the crib in the middle mattress setting, and adjust it to the low or high setting. RECALL: Jardin Cribs Sold by Babies R Us Due to Entrapment and Strangulation Hazards

The U.S. Consumer Product Safety Commission, in cooperation with Jardin Enterprises of Taiwan, today announced a voluntary recall about 320,000 Jardin Cribs. Consumers should stop using recalled products immediately unless otherwise instructed. The wooden crib slats and spindles can break, creating a gap, which can pose an entrapment and strangulation hazard to infants. There have been 42 incidents of crib slats and spindles breaking. Four children became entrapped in the space created by a broken slat or spindle. Two of the children had abrasions and bruising. Jardine wooden cribs in various styles and finishes, as listed below, are included in this recall.

RECALL: Nordstrom Girls' Sandals Due To Choking Hazard

The U.S. Consumer Product Safety Commission, in cooperation with the firm Nordstrom Inc or Seattle, Washington, today announced a voluntary recall about 1,500 Nordstrom’s Cadence-Lea and Trio-Lea Girl’s Sandals. RECALL: Remote-controlled Helicopter Toys Pose Risk of Fire

The U.S. Consumer Product Safety Commission, in cooperation with Westminste of Atlanta, Georgie, yesterday announced a voluntary recall about 102,000 “Thunder Wolf” Remote Controlled Indoor Helicopters. The rechargeable battery inside the helicopter can overheat. This can result in the helicopter’s body melting, as well as a risk of fire or burns to consumers. Westminster Inc. has received seven reports of overheating and/or melting of the helicopter, including one report of flames and two reports of minor property damage. No injuries have been reported. The “Thunder Wolf” remote-controlled indoor helicopter is made of foam and plastic, and measures about 7 inches by 3 inches. “TW0996” is printed on the tail of the helicopter and was sold in multiple accent colors. The helicopter comes with a controller and a separate charger. Only helicopters that do not have “Made in China” stamped on the underside of the helicopter are subject to the recall. Retail stores nationwide from June 2007 through December 2007 for about $20.
Consumers should immediately stop playing with the recalled helicopters and contact Westminster Inc. for a refund. Retailers will not accept returns or provide a refund.

Congressman Markey Proposes a Federal Ban on BPA

Representative Edward J. Markey (D-MA), a senior member of the House Energy and Commerce Committee with oversight over the Department of Health and Human Services, will introduce the “Ban Poisonous Additives (BPA)” Act today to prohibit the use of the chemical bisphenol-A (BPA) in all food and beverage containers.
BPA is prevalent in plastic bottles and most baby bottles and the linings of canned food and beverages, but more than 100 scientific studies have shown that low doses of BPA in animals can lead to increases in cancer, brain damage, altered immune function and other health problems.
Rep. Markey said, "From baby bottles to cans of chicken soup, BPA is present throughout American life. But concerns about the effects of BPA on human health have been mounting, and we've reached a point where the risk is simply too high. For the sake of the health of every man, woman and child in America, the best course of action we can take right now is to completely ban BPA in food and beverage containers, especially because there are alternatives already available."
BPA is used to harden plastics and is associated with a broad spectrum of hazardous effects. Often used to make plastic bottles and as a coating in the linings of food and beverage cans, it has been shown to leach out of the containers into liquids and foods, and the Centers for Disease Control & Prevention found measurable amounts of BPA in the bodies of 93 percent of the U.S. population studied.
"Representative Markey's legislation is an essential step to improving the health of all U.S. citizens," said Jeanne Rizzo, R.N., executive director of the Breast Cancer Fund. "The presence of bisphenol-A in commonly used goods is shockingly prevalent, and the average consumer is unaware of what products to avoid. Americans expect and believe that their government is safeguarding their health, and this legislation is an important step in that direction."
Rep. Markey's BPA Act will ban BPA in all food and beverage packaging.
The bill has been endorsed by the Breast Cancer Fund, Consumers Union, the Environmental Working Group, Health, Education and Resources, Inc., Making Our Milk Safe, the National Research Center for Women and Families, the Public Interest Research Group, the Association of Reproductive Health Professionals and the Chicago Consortium for Reproductive Health in Minority Communities.

RECALL: Walgreens' Remote-Controlled Helicopter Toy Due to Fire and Burn Hazads

The U.S. Consumer Product Safety Commission, in cooperation with TWIE - Tradewinds Interntional Enterprises Inc. of Californa, today announced a voluntary recall of about 152,000 Sky Champion Wireless Indoor Helicopters. Consumers should stop using recalled products immediately unless otherwise instructed. The rechargeable battery contained inside the helicopter can catch fire, igniting the helicopter and nearby combustible materials. This poses a burn or fire hazard to consumers. TWIE has received two reports of helicopters catching fire. No injuries or property damage have been reported. This recall involves the “Sky Champion” wireless indoor helicopter. The helicopter comes with a transmitter that controls and recharges the helicopter. The helicopter is made of foam and plastic and measures about 7 inches by 3 inches. The transmitter measures about 6 inches by 4 inches. “BH26047” is printed on the tail of the helicopter. “WIC 551777” and the UPC code is 630990006005 are printed on the packaging.

RECALL: Children's Hooden Sweatshirts by Adio Footwear due to Strangulation Hazard

The U.S. Consumer Product Safety Commission, in cooperation with Adio Footwear of California, today announced a voluntary recall of about 300 Boy's Hooden Zip Fleece Sweatshirts. Consumers should stop using recalled products immediately unless otherwise instructed. The garments have a drawstring through the hood, which can pose a strangulation hazard to children. In February 1996, CPSC issued guidelines (pdf) to help prevent children from strangling or getting entangled on the neck and waist by drawstrings in upper garments, such as jackets and sweatshirts. The boy’s “Champ Custom” hooded zip fleece sweatshirt (style # 40305.HZB) is white with blue pin-stripes on the front and red panels on the sides. ADIO is spelled out across the front. The sweatshirt was sold in boys’ sizes: small, medium, large and x-large. They were sold at Tilly’s and Bob’s stores nationwide from October 2007 through December 2007 for about $40 and manufactured in China.
Consumers should immediately remove the drawstrings from the sweatshirts to eliminate the hazard, and return the item to the place of purchase or to Adio Footwear for a full refund.

Surface paint on the recalled shaker guiro instrument contains excessive levels of lead, violating the federal lead paint standard. Incidents/Injuries: None reported. The recalled shaker guiro is a ridged, cylinder-shaped percussion instrument that contains beads and is played by scraping a stick along the surface. The shaker guiro measures about 2” wide x 5” long. One end of the instrument is painted red and the other end is painted blue.
